What is Form Health?
Form Health is a virtual weight-management program that provides expert, science-based care to treat patients with obesity or who are overweight. Each patient works one-on-one with a dedicated clinical Care Team to develop a personalized weight-loss plan that addresses behavior change, nutrition, physical activity, and—when medically necessary—an FDA-approved prescription for medication.
All care is virtual, including regular video visits and messaging with your Care Team, along with tracking tools and educational resources in the Form Health app. All Form Health patients receive a connected scale (at no cost to them) to track their progress.
How do I get a weight-loss prescription through Form Health?
If you enroll with Form Health, prescriptions for weight-management medications may be covered by Workday health plans when medically necessary. The following FDA-approved medications may be prescribed:
- Medications indicated for chronic weight management, such as Wegovy® (semaglutide), Saxenda® (liraglutide), or Zepbound® (tirzepatide)
- Medications like Ozempic® (semaglutide) or Mounjaro® (tirzepatide), used for weight loss when type 2 diabetes is present
Note: Your Form Health provider will evaluate your insurance coverage to ensure that your recommended treatment plan will be covered by your health plan.
Will weight-management treatment that I receive from physicians unaffiliated with Form Health be covered?
If you are currently taking weight-management prescriptions, we encourage you to enroll with Form Health, but you are not required to do so. Your prescription will continue to be subject to prior authorization.
If you are NOT currently taking weight-management prescriptions, for weight-management treatment and/or medications to be covered, you must use Form Health. Everyone’s body is different, and commonly prescribed treatment options aren’t successful for everyone. It’s important that you have the support of a physician who specializes in weight management and a registered dietitian to guide you.
What if I’m currently enrolled in Calibrate and/or already taking a weight-loss prescription?
Form Health has replaced Calibrate. Form Health will contact all Calibrate participants to help transition. If you’re currently taking a GLP-1 for weight management, here’s what you need to know:
- Existing prescriptions from Calibrate can continue to be filled without a prior authorization through December 31, 2024.
- Existing prescriptions from your provider will continue to require a prior authorization.
- If you’re not currently prescribed medication for weight management, participation in Form Health’s obesity program is required to get those prescriptions, effective January 1, 2025.
Note: GLP-1 prescriptions used to treat type 2 diabetes aren’t required to be prescribed by Form Health providers.

Will I be required to use Form Health to obtain my GLP-1 prescription to manage my type 2 diabetes?
No, your prescription can be filled as usual. However, you’re welcome to use Form Health services to assist you with weight management if you’d like.
How much will Form Health cost me?
Beside the cost of any weight management prescriptions, or missed appointments (if applicable), Form Health is available at no cost to you and your qualified dependents. Program fees are 100% paid by Workday, and virtual visits are 100% covered as preventive care under your Cigna or SimplePay Health medical plan.
Note: When you sign up for Form Health, you’ll be asked to provide your credit card information. This will be used in the event you miss an appointment without providing 48 hours’ notice. The missed appointment fee is $50 (for the first instance, your fee is waived).
Form Health sends you a connected scale, provided at no cost to you, as part of the Form Health program. If you require a weight-management prescription, your out-of-pocket costs for the medication will follow the cost-sharing terms of your medical plan.
Who qualifies for Form Health?
Form Health is a clinical care program for patients who are enrolled in Workday’s Cigna or SimplePay Health medical plan and are 18 or older, with the clinical condition of overweight or obese.
There are two programs available, which are dependent on the following criteria:
- Obesity program requires a BMI of 30 or higher or a BMI of 27 or higher, with a weight-related medical condition like high blood pressure, diabetes, etc.
- Overweight program requires a BMI of 25 or higher (this program will be available starting January 1, 2025).
Form Health also requires that you must have a primary care physician and have visited them within the last 12 months.

Certain serious conditions are incompatible with Form Health.
How do I sign up for Form Health?
Easy! Just visit their website to take a quiz and verify your eligibility for the program. Once you’re verified, follow the prompts to sign up.
